The Total CDM and CCMP billings metric focuses primarily on the GP-driven item numbers in terms of calculating the portion of billings derived from Chronic Condition Management services.

The item numbers included in this list come from schedules A15 and A17 of the Medicare Benefits Schedule.
The CDM and CCMP item numbers included for this metric are as follows:

CDM and CCMP billings also include Individual Allied Health Services (Items 10950, 10951, 10952, 10953, 10954, 10956, 10958, 10960, 10962, 10964, 10966, 10968, 10970, 93000 and 93013) for Chronic condition management.
There is also the option to include:
- Health Assessment
- Medication management reviews
- Nurse items
The default option on the page is to only include CDM and CCMP Items and Health Assessments, but if you want to include the other options you can select them from the drop-down filter list for fuller picture of CCM in your practice.
