DVA CVC Program

This metric shows the number of DVA Gold and White Card holders who (based on historical billings) may have a chronic disease and have not had a CVC item billed in the past 90 days.

The CVC Program proactively manages Veteran Gold Card holders with chronic conditions and Veteran White Card holders with DVA-accepted mental health conditions. Providers and participants work as a team to improve their health care in a general practice setting.

Possible CVC billings

This value shows the value of billings that could be billed from the patients who may be eligible for a CVC. The total is based on the MBS fee schedule shown beneath from your billings last month. This will not take into account any gap fee that your practice may charge. 

 

 

NOTE: The total is based on the MBS fee schedule shown beneath from your billings last month. If you have not billed a CVC item last month this will show as "No recent billing history to use for billing prediction" as in the screenshot above.

Possible patients CVC Program

This metric shows the number of DVA Gold and White Card holders who (based on historical billings) may have a chronic disease and have not had a CVC item billed in the past 90 days. Patients are deemed as possibly having a chronic disease if they have previously had a chronic disease management item billed.

List of patients for possible CVC

This list details the DVA Gold and White Card holders who (based on historical billings) may have a chronic disease and have not had a CVC item billed in the past 90 days. Patients are deemed as possibly having a chronic disease if they have previously had a chronic disease management item billed.

This table shows:

  • The date, time and appointment information of the next upcoming appointment for the patient
  • DVA card status: Gold or White card
  • Days since the last CVC billing (90 days and over)
  • Last attended appointment date
  • Most recent CDM item billing
  • Last doctor 
  • Usual doctor

From 1 July 2021, the DVA expanded the CVC Program to include white card holders with DVA-accepted mental health conditions holders in addition to gold card holders.
